Transitions
SUPPORTIVE HOUSING PROGRAM

Transitions, the YWCA Jamestown supportive housing program, provides safe and affordable housing and service-enriched case management to women and children who are homeless or at risk of being homeless. The program offers opportunities for residents to identify their capabilities and to develop the new skills necessary to be successful at healthy independent living.

Transitions is the single communal supportive housing program in Chautauqua County. The residential facility is equipped to house 14 adult women (18 years of age and older), and their children, and boys 12 years of age and under. Women and their children share sleeping quarters, common living room, bathroom, and kitchen areas. One of the strengths of the communal setting is the relationships women develop with each other, as well as with the staff. These relationships help to restore trust and a desire for change in a population that would otherwise remain lacking in these areas. Supportive services increase the residents' ability to build and maintain stronger relationships with employers, landlords, and the community as a whole.

Homelessness occurs for many reasons:

  • Domestic Violence

  • Substandard Housing

  • Loss of Income

  • Post-hospitalization

  • Post-substance Abuse Rehabilitation

  • Family Treatment Court; City Drug, Mental Health and Veteran's Court, Family Court, Drug Treatment

  • Post-incarceration

  • Relocation

  • Young Adult Pregnancy/Parenting

  • Eviction

  • Lack of Independent Living Skills


*Staff assists with every aspect of transitioning from homelessness to self-sufficiency, financial independence and safe permanent housing.

Transition's mission is to reduce homelessness through successful recognition, intervention and empowerment.

Residents receive support and assistance from community resources and YWCA staff through the following on-site services:

  • Crisis Intervention

  • Housing Needs Assessment

  • Information/Referrals/Follow Up

  • Case Management

  • Safety Assessments

  • Personal and Legal Advocacy

  • Domestic Violence Support Group

  • Assistance with Educational Goals

  • Assistance with Employment Goals

  • Landlord/ Tenant Negotiations

  • Family Activities

  • Budgeting and Smart Shopping

  • Educational Workshops


Transitions is a communal living facility for:

  • Women age 18 or older

  • Women with or without children

  • Non-custodial mothers

  • Pregnant teens 18 years of age and older

 

Additional Information:​

  • Minimal Rent includes all utilities

  • Secure building

  • Laundry facilities

  • Handicapped accessible

  • Referrals to the program are accepted, but not required



Thanks to the United Way of Southern Chautauqua County, and the New York State Office of Temporary and Disability Assistance for funding the YWCA Transitions program which helps the YWCA provide supportive housing for women and children in our community.
