Housing instability can have significant impacts on health and well-being, and through the New York State 1115 Medicaid Waiver program, we help address these critical needs as part of a comprehensive approach to health care.
What We Offer
Personalized Housing Search Assistance: We help connect clients with rental properties that fit their needs, preferences, and budgets.
Application and Documentation Support: Our team guides clients through the rental application process to improve chances of success.
Utility Assistance: Assistance with utility setup and payment support to help reduce barriers to stable housing.
Tenancy Support: Ongoing support to help maintain housing stability and avoid eviction or housing loss.
How It
Works
This program is part of the Health-Related Social Needs (HRSN) services under the NYS 1115 Medicaid Waiver, which funds non-medical supports that improve health outcomes by addressing social determinants of health, including housing. We work closely with regional Social Care Networks (SCNs) and healthcare partners to coordinate services, ensuring that participants receive holistic support tailored to their unique situations.
